Position: Registered Nurse (RN) - Home Health

Location: Deer Park, NY

Responsibilities:

• Deliver skilled nursing care to patients in need, as prescribed by physicians and supervised by the Clinical Services Supervisor.
• Efficiently manage the care of each patient to achieve positive outcomes, taking responsibility for the overall plan of care and ensuring the quality of care provided by other nursing staff (LPN, HHA) under your direction.
• Provide education to the patient's family and other members of the nursing team as needed.
• Plan, provide, and coordinate high-quality patient care.

Requirements:

• Hold a current license as a Registered Professional Nurse.
• Preferably have at least 2 years of acute care nursing experience and/or previous experience in home care nursing.
• Strongly prefer experience with OASIS.
• Preference for experience in medical-surgical care and wound care.
• Must possess an Associates Degree from an accredited college; a Bachelor's degree is preferred.
