Registered Nurse (RN) - Home Health

Responsibilities:

• The Home Care Nurse will provide skilled nursing care to patients in need of professional nursing services as prescribed by the physician and directed by the Supervisor of Clinical Services.
• Efficiently manage and oversee the care of each patient to ensure positive patient outcomes. This includes developing and implementing the overall plan of care and supervising other nursing staff (LPN, HHA) as needed.
• Educate patients' families and collaborate with other members of the nursing team.
• Responsible for the planning, coordination, and delivery of high-quality patient care.

Requirements:

• Current and valid Registered Nurse license.
• Preferably 2+ years of experience in acute care nursing and/or previous home care nursing experience.
• Strongly prefer experience with OASIS documentation.
• Preference for experience in medical-surgical nursing and wound care.
• Associate's degree from an accredited college required; Bachelor's degree preferred.
