We are looking for a dedicated and skilled Registered Nurse (RN) to join our team as a full-time Home Health Visiting Nurse. As an RN, you will provide comprehensive nursing care to patients in the comfort of their own homes in Danville, WV. The ideal candidate should have at least one year of experience as an RN in a home health setting or a related healthcare field.

Responsibilities:

• Conduct thorough patient assessments in the patients' homes and create personalized care plans based on their individual needs.

• Administer skilled nursing care to patients, which includes wound care, medication management, and other necessary nursing procedures.

• Educate patients and their families on effective health management and disease prevention strategies.

• Work collaboratively with members of the healthcare team, including physicians, therapists, and social workers.

• Maintain accurate and timely documentation of patient care in electronic medical records.

• Stay updated on current nursing practices and trends in home health care.

Qualifications:

• Must hold an active RN license in the state.

• Minimum of one year of experience in home health care or a related healthcare field.

• Possess excellent communication and interpersonal skills.

• Ability to work independently and manage multiple patients effectively.

• Reliable transportation, a valid driver's license, and auto insurance are required.

• Comfortable using electronic medical records.

• Must have a current CPR certification.
