Registered Nurse (RN) - Home Health

Responsibilities:

• Deliver Skilled Nursing Care:

  As an RN Case Manager, you will provide professional nursing services to patients based on the physician's orders and under the supervision of the Supervisor of Clinical Services.

• Efficiently Manage Patient Care:

  Your role will involve managing each patient's care to ensure positive outcomes. This includes developing and implementing care plans and overseeing the delivery of care by other nursing staff (LPN, HHA) whom you will supervise and direct.

• Provide Education:

  Teach patients, families, and other members of the nursing team to enhance their understanding of healthcare procedures and promote optimal care.

• Coordinate Quality Care:

  Take charge of planning, providing, and coordinating high-quality patient care to meet individual needs and improve overall patient well-being.

Requirements:

• Registered Professional Nurse:

  Must hold a valid license as a Registered Professional Nurse.

• Prior Nursing Experience:

  Preferred 2+ years of acute care nursing experience and/or previous experience in home care nursing.

• OASIS Experience:

  Strongly preferred experience working with OASIS (Outcome and Assessment Information Set).

• Specialized Skills:

  Preference for candidates with knowledge and experience in med-surge and wound care.

• Educational Qualification:

  Must have an Associates Degree from an accredited college, with a preference for those holding a Bachelor's degree.
