What we're seeking in the ideal candidate:
•Completion of a social work degree program from an accredited college or university (BSW degree required).
•Valid licensure as a LISW or SW in the state of Ohio.
•Prior experience in hospital social work.
•Preferably, experience in discharge planning within a hospital setting.
•Familiarity with CMS Conditions of Participation for discharge planning is a plus.
Hours and compensation:
•This is a full-time position primarily on the day shift (Monday to Friday, 7:30am-4:00pm), with every 4th weekend duty.
•Compensation ranges between $47K and $70K, depending on experience, and includes a comprehensive benefits package.
Your main responsibilities:
•Collaborating with other healthcare professionals, you will provide, coordinate, and facilitate an appropriate plan for patients during hospitalization, ED visits, and visits to the Center for Cancer Care, when needed.
•Using critical thinking skills, you will organize and ensure optimal transitions of care for patients, operating within provider guidelines and limitations.
•Offering emotional and environmental support to patients, family members, and visitors, you will also assist in connecting patients with complex needs to necessary resources.
•Adhering to organizational policies and procedures, regulatory/accrediting body requirements, and professional practice standards.
•You will assess, evaluate, and re-evaluate the physical, psychosocial, and behavioral needs of patients.
•In collaboration with the patient, their family or designated caregiver, and the healthcare team, you will establish and coordinate an appropriate discharge plan.
•Working effectively, you will obtain referrals and prior authorizations from external agencies to ensure a safe and suitable level/transition of care (such as APSI, Home Health Care, DME, SNF, Hospice, LTAC, Acute Rehab, and/or Chronic Care/Transition Care Team).
•You will also make referrals to the relevant community agencies based on individual situations/cases (for example, Behavioral Health Partners, The Department of Job and Family Services, Addiction Services, Adoption Services, etc.).

•The role reports to a highly respected Director.
