POSITION SUMMARY:
-
As a member of the interdisciplinary team, the Case Manager identifies the psychosocial and environmental needs of patients and their families throughout the hospitalization process. They provide evidence-based care while ensuring efficient utilization of hospital resources. The Case Manager collaborates with other team members to develop and implement a comprehensive discharge plan that promotes positive patient and family outcomes. They also refer patients to community services, offer emotional support, provide therapeutic interventions, conduct crisis intervention, offer bereavement services, and educate patients and families on their healthcare options. Additionally, the Case Manager supervises graduate social work student interns and assists patients in accessing primary and preventive care services instead of the emergency department. They also educate patients on available transportation resources in the community.

Minimum:

Master's degree in Social Work from an accredited program.

Preferred:

Licensed Clinical Social Worker.

Licensed Master Social Work (LMSW) or Licensed Clinical Social Worker (LCSW) in NY State or eligible for NY State License.

PROFESSIONAL CERTIFICATIONS:

Screen certification or experience or intent to pursue within six months.

WORK EXPERIENCE:

Minimum:

Two years of relevant experience in healthcare.

Experience in counseling and discharge planning.

Preferred:

Prior experience in an Acute Hospital setting and/or knowledge of Nursing Home operations. Previous navigator experience and conducting Crisis Intervention Evaluations.

Minimum:

Ability to handle confidential information with discretion. Strong interpersonal, communication, and organization skills. Proficient in computer literacy.

Experience with Microsoft Office products and electronic medical record systems.
