We're seeking a compassionate and experienced Social Worker / LCSW to join our interdisciplinary care team in Eureka. In this pivotal role, you’ll support older adults through comprehensive care planning, psychosocial support, and resource coordination—helping them live with dignity, support, and purpose.

What You’ll Do:

• Conduct psychosocial assessments and contribute to individualized care plans

• Deliver and document services including counseling, case management, and care transitions

• Coordinate care across hospital, home, and skilled nursing settings

• Serve as a liaison between participants, families, caregivers, and care teams

• Lead care conferences and help plan discharges when needed

Preferred Experience:

• Background in behavioral health or substance use counseling

• Familiarity with interdisciplinary teams and elder care models
