🔑 Must-Haves:

• Master’s degree in Social Work (MSW)

• Active Ohio license: LSW, LISW, LPC, or LPCC

• At least 1 year of professional social work experience preferred

💰 Schedule & Compensation:

• Full-time day shift with rotating weekends

• Salary Range:

  • MSW: $56,000 – $78,000

  • BSW: $51,000 – $70,000

• Full benefits package included

💼 What You’ll Be Doing:

• Deliver professional social work services to hospice patients and families, supporting overall health, emotional wellbeing, and end-of-life planning

• Conduct patient assessments and develop care plans focused on comfort, dignity, and psychosocial support

• Provide crisis intervention, supportive counseling, and education for patients and caregivers coping with illness, grief, or disability

• Collaborate with an interdisciplinary hospice team to ensure personalized and compassionate care

• Participate in community engagement initiatives, stay informed on relevant laws, and uphold ethical standards of practice

• Document in EPIC EMR and contribute to ongoing quality care standards
