Speech Language Pathologist – CFY Welcome

- LOCATION: Williamsville, NY (suburb of Buffalo NY).
- As a member of the Speech-Language Therapy team, you will play a crucial role in ensuring the health and well-being of our residents. You will be responsible for evaluating and developing a personalized speech-language therapy treatment plan, if necessary, conducting audiometric screenings, and providing appropriate recommendations for further audiology services. Your diligence and dedication will ensure that our residents receive the highest quality of care.
Responsibilities:
1. Conduct screenings and evaluations for residents with communication problems as requested. Develop and document therapy plans of care if necessary.
2. Coordinate and administer audiometric screening tests for residents and make appropriate referrals.
3. Initiate and implement speech-language therapy programs based on physician orders. Maintain regular communication with physicians throughout the therapy program.
4. Treat residents with respect and kindness, upholding their rights at all times.
5. Maintain accurate records including evaluations, progress notes, and discharge summaries for residents participating in speech-language treatment programs.
6. Collaborate with other staff members to integrate speech-language treatment programs with overall resident care goals.
7. Attend Resident Care Planning and Family/Resident Care Plan Review meetings related to speech-language treatment programs.
8. Ensure accurate record-keeping of statistical reports, in-service education information, budget data, and equipment inventory.
9. Prepare and submit statistical reports for Medicare billing and complete assessment forms as required.
10. Attend staff meetings and participate in in-service training sessions.
11. Assist in creating and conducting in-service training programs for facility personnel involved in resident care.
12. Regularly review and update written speech-language policies and procedures.
13. Manage program costs within the allocated budget.
14. Adhere to safety rules and practices to ensure the well-being of staff, residents, and visitors.
15. Generate innovative ideas to enhance resident care quality and improve position productivity.
16. Meet service excellence standards and goals set for this position and the facility.
17. Provide exceptional customer service to residents, staff, and visitors.
18. Perform other assigned duties from supervisors, management staff, or the Administrator.
Qualifications:
• Hold a Master's degree in Speech-Language Pathology from an approved institution.
• Possess a valid New York State license to practice as a Speech-Language Pathologist.
• Preferably hold a Clinical Competence Certification from the American Speech and Hearing Association.
• Excellent verbal and written communication skills.
• Ability to work collaboratively and professionally with others.
• Valid qualification verification from the New York State Department of Health, Office of Rehabilitation Therapies (license number/certification number).
• Consistently perform tasks to high standards of excellence.
