LOCATION: Williamsville, NY (situated in the suburbs of Buffalo, NY)

SETTING: Skilled Nursing Facility

Full-Time, Weekdays. Occasional weekends and holidays may be required.

  • As a member of the Speech-Language Therapy team, you will play a crucial role in ensuring the health and well-being of our residents. Your responsibilities include assessing residents for communication problems, creating therapy plans, conducting audiometric screenings, and collaborating with physicians to ensure the highest quality of care for our residents.

Responsibilities:

1. Conduct screenings and evaluations for communication problems in residents upon request.
2. Coordinate and facilitate audiometric screening tests and referrals for audiology evaluations if necessary.
3. Create individualized speech-language therapy plans based on physician orders and maintain regular communication with physicians throughout the therapy program.
4. Treat residents with respect and kindness, ensuring their rights are fully observed.
5. Keep written evaluations, progress notes, and discharge summaries for all residents in the speech-language treatment program.
6. Collaborate with other staff members to meet the special needs of residents and integrate speech-language treatment with other disciplines.
7. Participate in Resident Care Planning and Family/Resident Care Plan Review meetings for residents in the speech-language treatment program.
8. Maintain records including statistical reports, in-service education information, budget details, and material/equipment inventory.
9. Complete statistical reports for Medicare billing payments and assessment forms as required.
10. Attend staff meetings and participate in in-service training sessions as needed or preferred.

11. Assist in developing and conducting in-service training programs for facility personnel involved in resident care.
12. Review and revise written policies and procedures for speech-language therapy on an annual basis.
13. Manage program costs within the annual budget.
14. Maintain a safe working environment and adhere to safety rules and practices.
15. Continuously generate and develop ideas to enhance resident care and improve productivity.
16. Uphold the service excellence standards and goals established for this position.
17. Provide exceptional customer service to residents, staff, and visitors.
18. Perform any other duties assigned by supervisor, management staff, or Administrator.

Qualifications:

• Must have an M.A. or M.S. Degree in Speech-Language Pathology from an approved school.
• Valid New York State license to practice as a Speech-Language Pathologist.
• Preference given to candidates with Clinical Competence Certification from the American Speech and Hearing Association.
• Strong verbal and written communication skills.
• Ability to work collaboratively and diplomatically with others.
• Verification of qualifications by the New York State Department of Health, Office of Rehabilitation Therapies (license number/certification number).
• Proven ability to meet established standards of excellence.
