Nurse Case Manager (Hospice - RN)

We’re seeking compassionate and experienced Registered Nurse Case Managers to join our growing hospice team in the Chandler, OK area. If you’re an RN with a minimum of 1 year in hospice or home health, who thrives in a fast-paced, independent environment, we want to hear from you.

Must Haves:

• Active RN license in the state of Oklahoma

• 1+ year of hospice or home health experience (REQUIRED)

• Proven ability to manage multiple priorities and work autonomously

• Strong communication and coordination skills

• EMR experience is a plus!

Open Positions:

1. Stroud, Prague, Bristow, Meeker Area

2. Chandler, Agra, Cushing, Stillwater Area

3. Weekend RN – Admissions & backup support

What You’ll Do:

• Provide skilled nursing care to hospice patients in their homes or care facilities

• Coordinate care across the interdisciplinary team, patients, and families

• Educate patients and caregivers on disease processes, medication management, and care plans

• Complete admissions and maintain accurate, timely documentation for Plan of Care and IDT meetings

• Serve on-call rotation and provide telehealth support as needed

• Ensure regulatory compliance and high-quality care delivery
