Nurse Case Manager (Hospice RN)

Location: Chandler, OK

We are seeking compassionate and skilled individuals with hospice expertise to join our dedicated team as Nurse Case Manager (Hospice RN).

Candidates must have a minimum of one year of hospice experience and demonstrate the ability to effectively manage a fast-paced and independent workload.

Responsibilities:

• Conduct professional nursing visits to patients receiving hospice care in their homes or facilities
• Facilitate coordination of care among the interdisciplinary team, patients, and their families
• Provide on-call support as required
• Perform and coordinate both field and telemedicine visits, adhering to regulatory guidelines and patient needs
• Educate patients and their family members / caregivers about available services and ensure compliance with the established care plan
• Train and educate patients, family members / caregivers, and staff on the appropriate use of medications, durable medical equipment (DME), and supplies
• Complete patient admissions as necessary
• Maintain compliance by completing all Plan of Care and Interdisciplinary Team paperwork

Qualifications:

• Registered Nurse with a degree in Nursing
• Active RN License in the state
• Minimum of one year of experience in hospice or home health
• Demonstrated exceptional interpersonal and communication skills
• Prior experience with electronic medical records (EMR) software for hospice and/or home health is a bonus
