  • Our physical therapy staff members play a vital role in ensuring the health and well-being of our residents by delivering high-quality therapy services. A valid physical therapy license specific to the state is required.

Responsibilities:

  • Initiate physical therapy treatments as per physician's orders and maintain necessary communication with physicians.

  • Create written evaluations, progress notes, and discharge summaries for residents enrolled in the physical therapy program.

  • Collaborate with other healthcare professionals to address the special needs of residents, enhancing their rehabilitation or maintenance of function.

  • Provide input for Resident Care Planning meetings and attend them in the absence of the Director of Physical Therapy.

  • Maintain statistical records of residents' treatments.

  • Assist in organizing and conducting in-service training programs upon request.

  • Update and maintain residents' plan of care in the Interdisciplinary Kardex.

  • Submit supply requisitions needed for residents' treatment to the Director of Physical Therapy.

  • Assume all duties of the Director of Physical Therapy during absences, including supervision of Physical Therapy Aides and fulfilling necessary documentation requirements.

Qualifications:

  • Completion of an accredited physical therapy school is required.
  • A valid state license to practice physical therapy is mandatory.
  • New graduates from physical therapist programs are welcome.
  • Preference given to candidates with two years of experience in physical therapy.
  • Interest in physical therapy treatment programs for the geriatric and subacute populations is essential.
  • Ability to provide guidance and direction to others is necessary.
