Hospice RN Clinical Liaison, Hospital Referrals

Manchester, NH (Field-based, Bedford and Manchester hospitals)

Use your bedside nursing background to build hospital referral relationships for a hospice provider. This is a hospital-focused liaison role, not bedside nursing, on a Monday to Friday, 8:00 AM to 4:30 PM schedule. Pay is $100K to $115K.

Must-Haves

• Active, unencumbered RN license
• Hospital bedside nursing background, including med-surg
• 1+ year of healthcare sales or marketing
• Able to build relationships with hospital teams, discharge planners, case managers, and physicians
• Valid driver's license, auto insurance, and dependable transportation

What You'll Do

• Build and maintain relationships with hospital referral sources
• Make planned calls on physicians, case managers, discharge planners, and social workers to grow census
• Educate patients, families, and clinical teams on hospice services
• Support referrals, eligibility discussions, and the admission and consent process
• Drive hospital referral activity and hospice census growth

Nice-to-Haves

• Experience in Bedford or Manchester-area hospitals
• Hospice nursing or 1 to 2 years of hospice or palliative marketing
• BSN or related healthcare degree
