Internal Medicine Nurse Practitioner, Outpatient

Coram, NY | $200,000 to $300,000 per year | Full-Time or Part-Time | On-Site

A physician-led internal medicine and primary care group with multiple locations across Long Island is seeking a Nurse Practitioner to join its Coram practice. The group has an established presence across the Long Island market with a focus on preventive, acute, and chronic disease management in a fully outpatient setting. This is a 100% outpatient position with no call, no hospital responsibilities, and a Monday through Friday schedule with flexible hours.

Schedule

Monday through Friday, day shift. Full-time or part-time hours available with flexible scheduling. No call. No evenings. No weekends.

Compensation

$200,000 to $300,000 annually, commensurate with experience and hours.

What You'll Do

The Nurse Practitioner functions as an independent clinical provider within a physician-led internal medicine practice, delivering direct patient care across preventive, acute, and chronic disease management in an outpatient primary care setting. The NP develops and implements individualized treatment plans, provides patient education on diagnoses, treatment options, and preventive strategies, and refers patients to appropriate specialty services as needed.

Documentation is completed within the practice EMR system in compliance with HIPAA, NYS DOH, CMS, and applicable payer guidelines. The NP collaborates with referring physicians, specialists, and care teams to coordinate patient management and provides clinical oversight to nurses, medical technologists, and support staff within the practice. The role includes participation in population health initiatives and ongoing quality improvement projects, as well as contribution to patient panel development aligned with practice growth objectives.

Required Qualifications

• Bachelor of Science in Nursing (BSN) and Master of Science in Nursing (MSN) from an accredited program
• Active New York State Registered Nurse (RN) license
• Active New York State Nurse Practitioner (NP) license
• National board certification as a Family Nurse Practitioner (FNP-BC or FNP-C) or Adult-Gerontology NP
• Training or clinical experience in internal medicine or primary care
• Minimum 8 years of experience as a practicing Nurse Practitioner
• Current BLS certification
• Demonstrated ability to build and maintain a patient panel in an outpatient setting

Preferred Qualifications

• Prior experience in an outpatient internal medicine or primary care practice on Long Island
• Familiarity with value-based care metrics, population health programs, and quality reporting requirements
• Experience with EMR documentation in a high-volume outpatient setting
• Interest in or experience with clinical leadership responsibilities
• Established patient or referral relationships on Long Island
