- Setting: Specialized Nursing Facility
- Our team of physical therapy professionals plays a crucial role in the overall health and well-being of our residents by providing comprehensive and high-quality therapy services. A valid Physical Therapy license issued by the state is required.
- Responsibilities:

Execute skilled physical therapy treatments according to physician's orders while maintaining regular communication with the physician.

Document evaluations, progress notes, and discharge summaries for residents participating in the physical therapy program.

Collaborate with other healthcare staff to address the unique needs of residents and enhance their rehabilitation or functional maintenance.

Provide input during Resident Care Planning meetings and serve as a substitute when the Director of Physical Therapy is absent.

Maintain records of residents' treatment progress for statistical purposes.

Assist with in-service training programs as needed.

Update and maintain the Inter-disciplinary Kardex to reflect residents' individual care plans.

Submit requisitions for necessary supplies to the Director of Physical Therapy.

Assume all responsibilities of the Director of Physical Therapy in their absence, including supervising Physical Therapy Aides and completing necessary documentation.
- Qualifications:

Completion of an accredited physical therapy program is required.

Must hold a state license to practice physical therapy.

Preference given to candidates with two years of experience in physical therapy.

Interest in physical therapy treatment programs for geriatric and subacute populations is essential.

Ability to lead and direct others is necessary.
