Job Summary:

  1. Evaluate and assess Residents' conditions upon admission, then develop and implement personalized therapy plans to manage their conditions, prevent further disabilities, and promote long-term health benefits.
  2. Main focus is on improving mobility, reducing pain, and preventing disability through exercise therapy, manual therapy, and other techniques.

Primary Responsibilities:

  • Conduct thorough evaluations of Residents' conditions and create appropriate rehabilitation plans.
  • Assist Residents in enhancing their mobility.
  • Relieve Residents' pain through exercises, hands-on therapy, stretching, and utilization of physical therapy devices.
  • Provide guidance to Residents on the proper use of physical therapy equipment.
  • Aid Residents in their tasks and monitor their performance.
  • Inform Residents about the anticipated stages of their recovery and guide them on how to handle each situation.
  • Maintain confidentiality of Resident information to ensure their confidence and protect facility operations.
  • Supervise physical therapy assistants as required.
  • Create and maintain a safe and clean working environment.
  • Adhere to infection-control policies and protocols to safeguard Residents and employees.
  • Ensure compliance with all federal, state, and local legal and professional requirements.
  • Collaborate with interdisciplinary team members in providing optimal care for Residents.

Required Qualifications:

  • Graduate of an Accredited Physical Therapy Program.
  • Current PT License in the State of New York.
  • Prior experience in skilled nursing or long-term care is advantageous.
