Job Summary:

  • Responsibilities include providing a wide range of rehabilitation services, including assessments, treatment planning, and therapeutic interventions within an interdisciplinary setting. Ensure adherence to professional practices, ethical standards, and contribute to program development, quality improvement, problem-solving, and productivity enhancement.

Education, License & Certification:

  • Minimum requirement is a four-year Bachelor's degree from an accredited program (or foreign equivalent). State licensure is mandatory for the role, along with a valid BLS Certification.


  • Previous experience or training in assisting patients with disabilities or injuries throughout their lifespan.

Primary Responsibilities:

1. Display professionalism and maintain good communication with patients, families, colleagues, and others.

2. Perform initial assessments using appropriate tools and adaptive equipment/materials suitable for the patient's age and developmental stage. Ensure accurate documentation of assessment results.

3. Develop individualized treatment plans with specific objectives and goals related to functional deficits. Choose appropriate interventions based on assessment outcomes, considering patient's age, cognitive abilities, and developmental level. Administer medication as directed.

4. Evaluate patient response to treatment, adjust and update treatment plans accordingly. Communicate progress, program planning, and discharge plans to patients and their families throughout the rehabilitation process.

5. Document patient care, progress, and treatment effectiveness in compliance with policies and procedures. Provide necessary information to referring providers regarding treatment outcomes and further clinical recommendations. Ensure accurate charge capture. Complete discharge summaries as required.

6. Educate patients and families to enhance functional independence. Involve family members in treatment, education, and discharge planning to ensure a safe transition home and continued care. Adapt educational methods to meet individual needs. Collaborate with the multidisciplinary team in determining the next level of care.

7. Participate in continuing education and professional development activities. Attend/present at in-service sessions and external growth opportunities to share knowledge with the team.

8. Adhere to federal and state guidelines, professional organizations, entity facilities, and organizational policies. Familiarize and comply with applicable professional codes, employee handbooks, unit policies, and procedures.

9. Meet established productivity standards in a fiscally responsible manner.

Additional Duties:

  • Perform all required tasks within the Physical Therapy Department as specified in the Competency Checklist.
