Position Overview:

Key Responsibilities:

• Conduct comprehensive assessments of patients' health conditions in their home settings

• Develop and implement individualized care plans in collaboration with multidisciplinary healthcare teams

• Administer medications and prescribed treatments with accuracy and care

• Educate patients and families on health conditions, treatment plans, and preventive care strategies

• Monitor patient progress and adjust care plans as needed to meet evolving health goals

• Maintain accurate, timely, and compliant documentation of patient care and progress

• Adhere to all regulatory guidelines and uphold high standards of clinical practice

Qualifications:

• Current and active Registered Nurse (RN) license in the state of Minnesota

• Previous Home Health nursing experience is required

• Strong clinical assessment and decision-making skills

• Excellent communication and interpersonal abilities to foster trust with patients and families

• Self-motivated and capable of working independently in a home-based care setting
