Hospice Admissions Nurse

Admission Nurse (Hospice)
Work Schedule: Full-Time, Monday-Friday (11am - 8pm)
Job Overview:
As an Admission Nurse (Hospice), your responsibilities will include conducting comprehensive patient assessments, facilitating smooth admission processes, and providing compassionate care coordination to ensure the utmost support for hospice patients and their families.
Role and Responsibilities:
- Effectively coordinate and supervise all aspects of new patient admissions
- Collaborate with patient care teams to ensure seamless communication
- Educate families on patient conditions, needs, and treatment
- Perform thorough head-to-toe physical assessments
- Complete documentation promptly and accurately
- Other duties as assigned
Education: Must be a graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current State license as a Registered Nurse; Current driver’s license. Current CPR Certification.
Experience: A minimum of two (2) years' experience as a Registered Nurse in a clinical care setting and at least one (1) year of full-time experience in hospice care.
Skills: Possess nursing skills as defined by generally accepted standards of practice. Knowledge of state and federal regulations is required. Strong interpersonal skills are a must.
Transportation: Reliable transportation and valid auto liability insurance are necessary.
