The Role of a Hospice Admission Nurse

Work Schedule: Full-Time, Monday to Friday (11am - 8pm)

Overview of the Position:

As a hospice admission nurse, your primary responsibilities will revolve around conducting comprehensive assessments of patients, facilitating a smooth admission process, and providing compassionate coordination of care. Your goal is to offer the best possible support to hospice patients and their families.

Key Responsibilities:

• Manage and coordinate all aspects of new patient admissions
• Collaborate closely with patient care teams to ensure effective communication
• Educate families on patient conditions, needs, and care requirements
• Conduct thorough physical assessments
• Complete documentation promptly and accurately
• Perform any other duties as assigned

Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.

Licensure: Hold a current State license as a Registered Nurse, along with a valid driver’s license. Also, possess a current CPR Certification.

Experience: Have a minimum of two (2) years' experience as a Registered Nurse in a clinical care setting, including at least one (1) year in a full-time hospice role.

Skills: Demonstrate nursing skills aligned with generally accepted standards of practice. Possess knowledge of both state and federal regulations. Exhibits good interpersonal skills.

Transportation: Must have reliable transportation and a valid auto liability insurance policy.
