ADVANCED PRACTICE NURSE (APN)

We are currently seeking an experienced and highly skilled Advanced Practice Nurse (APN) to join our Adult Medicine clinic in Hartford, CT. In this role, the APN will provide comprehensive primary health care services to patients, including health promotion and disease prevention activities. Collaborating closely with our team of physicians, the APN will also refer patients to consultants when necessary for further diagnosis and treatment.

Key responsibilities:

• Graduate from an accredited nursing school and completion of an approved nurse practitioner program
• Valid State of Connecticut licensure as a nurse practitioner
• DEA and state narcotic registration or eligibility to obtain
• Extensive knowledge of nursing theory and practice or physician assistant theory and practice
• Proven track record of providing high quality, comprehensive clinical care to patients
• Ability to provide culturally competent care in a collaborative environment
• Skills in medical history taking, assessing medical conditions, and interpreting findings
• Excellent communication skills, both oral and written
• Ability to maintain accurate and timely clinical documentation
• Proficiency in making evidence-based clinical decisions and adhering to quality measures
• Familiarity with electronic health record and other computer systems
• Willingness to perform other assigned duties
• Preference for candidates with 1-2 years of experience in an ambulatory setting or urgent care
• All candidates must successfully pass pre-employment background check and drug screen
