Overview:

The Rehabilitation Specialist evaluates and assesses patients upon admission, devises and implements customized rehabilitation plans to manage their conditions, prevent further disability, and optimize long-term health outcomes.

The primary focus is to enhance mobility, mitigate pain, and prevent disability through a combination of therapeutic exercises, manual techniques, and other modalities.

Responsibilities:

• Evaluate patients' physical condition and develop tailored rehabilitation strategies.
• Assist patients in improving their mobility and range of motion.
• Alleviate patients' pain utilizing exercise therapy, hands-on techniques, stretching, and specialized equipment.
• Instruct patients on the proper usage of physical therapy devices and equipment.
• Guide and monitor patients during therapy sessions, providing necessary support and guidance.
• Educate patients on the different stages of recovery and equip them with coping strategies for each phase.
• Prioritize confidentiality to preserve patients' trust and maintain facility operations.
• Supervise physical therapy assistants when required.
• Promote a safe and clean working environment.
• Adhere to infection-control policies and protocols to ensure the well-being of patients and staff members.
• Comply with federal, state, and local legal and professional regulations.
• Collaborate with interdisciplinary teams to provide comprehensive care for patients.

Qualifications:

• Graduate of an accredited Physical Therapy Program
• Current New York State PT License
• Prior experience in skilled nursing or long-term care settings is preferred.
