Physical Therapist (Home Health PT)

 

Please note: Home health industry experience is required for this position, with no exceptions.

As a Physical Therapist (PT), you will provide exceptional care to patients in the comfort of their own homes. Collaborating with the physician and patient care team, you will manage the therapeutic care process for a specific group of patients. This includes conducting patient assessments, developing personalized care plans, and ensuring goal achievement, patient engagement, and a positive patient experience. Additionally, you will deliver or oversee a majority of the patient's physical therapy, offering vital support throughout the continuum of care.

Key responsibilities:

  • Conduct evaluations and perform treatment interventions adhering to established laws and professional standards.
  • Assess and evaluate therapeutic, rehabilitative, and functional status, collaborating on the development of comprehensive care plans.
  • Administer purposeful and skilled therapeutic care at patients' homes, utilizing techniques and procedures that align with the physician's prescribed plan of care.
  • Instruct patients, families, and caregivers on the use of therapies, exercises, and therapeutic devices for use outside of therapy sessions.
  • Prioritize caseload, determine daily staffing needs, and coordinate assignments and handoffs as necessary.

EDUCATION:

Must be licensed as a Physical Therapist in the State

 

QUALIFICATIONS:

· Current and valid state professional license in Physical Therapy.

· Minimum of one year experience as a physical therapist in a home care setting, with proficiency in conducting comprehensive assessments and OASIS-C knowledge preferred.

· Demonstrated ability to make sound clinical judgments and assess patient needs to develop individualized care plans.

· Proficient in essential clinical skills.

· Excellent written and verbal communication skills, with fluency in English.

· Reliable automobile for work, valid driver's license, clean driving record, and proof of insurance.

· Proficiency in using personal computers, including email, clinical software, word processing, and spreadsheet applications.
