Physical Therapist, PT – Full-time/Per Diem

We are currently looking for a skilled and experienced Physical Therapist to join our team. Our ideal candidate will provide top-quality patient care that will greatly improve the lives of our patients, all from the comfort of their own homes.

As a Physical Therapist, you will assess and treat residents based on physician orders. Your use of various therapeutic procedures will help restore muscle strength, coordination, body alignment, relieve pain, and ultimately improve movement and function. You will also collaborate with the interdisciplinary team, providing valuable support and information within the field of Physical Therapy.

Responsibilities

  • Conduct physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluations for clients under medically approved Plan of Care.
    • Complete initial assessments promptly, within forty-eight hours of referral or as arranged.
    • Comprehensively assess clients, including OASIS data set, within forty-eight hours of referral for speech therapy services or as arranged.
    • Develop therapy care plans to guide other health team members involved in the care plan. Regularly review and update plans based on Agency policy and client needs.
    • Thoroughly document all findings, plans, interventions, and outcomes, submitting documentation to the Agency within seven days or per Agency policy.
    • Effectively communicate plans and changes to physicians, client case managers, and other caregivers using care plans, progress notes, and active participation in care conferences.
    • Notify the office promptly of any schedule changes or physician orders.
  • Deliver appropriate education to clients based on their needs and learning abilities, considering their environment.
    • Assess clients' knowledge levels and identify learning needs.
    • Plan, teach, supervise, and counsel clients on their physical therapy needs to promote improvement or recovery, symptom management, and community resources.
    • Evaluate the effectiveness of the teaching plan and establish care plans for others involved in client education.
    • Participate actively and contribute to interdisciplinary team meetings and other required meetings.
    • Share areas of expertise across teams and initiate interdisciplinary referrals when necessary.
    • Participate in discharge planning and document it in client records.
  • Assist in the development, implementation, and evaluation of ongoing quality improvement processes.
    • Participate on the Clinical Record Review Committee.
    • Utilize the OASIS assessment tool and outcome reports to enhance therapy services and improve client outcomes.
    • Collaborate with the interdisciplinary team to ensure consistency in data collection methods.
  • Promote personal safety and maintain a safe environment for clients and coworkers.
    • Demonstrate knowledge of safety and infection control practices by complying with agency policies and procedures.
    • Recognize and respond to potentially unsafe situations.
    • Practice safely when using equipment.
    • Evaluate the safety of the environment and take initiative to prevent accidents and promote safety.
  • Supervise applicable home care team members.
    • Provide in-home supervision of home health aides and physical therapy assistants as directed and according to Agency policy.
    • Evaluate employee performance and conduct timely reviews as delegated.
  • Perform other related duties and responsibilities as deemed necessary.
  • Perform the job in compliance with Agency policies and procedures, as well as professional and community standards.
    • Provide services in accordance with applicable state, federal, CHAP, and JCAHO standards.
    • Attend meetings and required educational programs.
    • Take responsibility for personal and professional development, identifying learning and developmental needs to discuss with your supervisor.

Qualifications

  • Graduate from an American Physical Therapy Association-approved program, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
  • Hold a current license/registration as a Physical Therapist in the state(s) of practice.
  • Minimum of two years of experience in the healthcare field, preferably in a home care setting.
  • Current CPR certification.
  • Demonstrate strong verbal and written communication skills, as well as good interpersonal skills.
  • Show the ability to work independently with minimal supervision.
  • Must be a licensed driver with automobile insurance that meets state and/or Agency requirements, and possess a good driving record.
