PRIMARY CARE PHYSICIAN

We offer a competitive base salary of 250-300K with an annual incentive opportunity, paid relocation, full benefits, equity, and a signing bonus. This is a full clinical onsite role in Fresno, CA.

As a Primary Care Physician, you will be responsible for conducting comprehensive assessments and collaborating with our team to develop individual care plans for each participant. Under the guidance of our Medical Director, you will oversee the care of participants in nursing homes and hospitals, including admissions, discharges, and coordination with medical specialists. Additionally, you will provide guidance and support to Nurse Practitioners and other team members.

Here are the essential job duties:

• Conduct assessments, develop care plans, and provide primary care to participants
• Collaborate with community-based physicians and the Interdisciplinary Team (IDT) for discharge planning
• Participate actively in IDT meetings, communicate participant changes, and collaborate on care planning decisions for 24-hour care delivery
• Maintain care of participants in nursing homes through regular visits based on participant need
• Participate in rotating night and weekend call

Here is how you will make an impact:

· Interview, examine, and treat clinical patients for medical patient care

· Educate patients and families on wellness, prevention, and early detection

· Make referrals based on examination and patient needs

· Contribute to quality improvement, management, continuing education, and patient care programs

· Help resolve complaints, requests, and inquiries from patients

· Maintain confidentiality of all patient information according to federal guidelines and regulations

· Complete daily documentation of medical records (EMR)

· Cover night calls as required

Job Requirements:

• Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
• Doctorate Degree
• Board Certification in Internal Medicine or Family Practice or Geriatrics (preferred)
• Active California Physician License without Restrictions
• Active state CDS Certification and DEA
• Enrollment or eligibility for enrollment to provide services to Medicare and Medi-Cal participants
