Care Delivery Manager (Home Health - RN)

Full-Time | Direct Hire | Queens & Westchester, NY

We are currently seeking multiple full-time Care Delivery Managers (RN) to join our healthcare client’s growing team in Queens and Westchester, NY. These leadership roles are essential to supporting and overseeing Community Health Nurses who provide in-home care for geriatric participants across the five boroughs of NYC.

The ideal candidate will be a seasoned Registered Nurse with strong managerial experience, proven leadership in home health settings, and a passion for improving patient outcomes. Please note: these positions require an RN license and at least one year of experience in a certified home health agency (CHHA).

Key Responsibilities:

  • Oversee the development and execution of personalized care plans to enhance participant satisfaction and health outcomes.

  • Lead and facilitate Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDR), and other care coordination meetings, ensuring timely completion of attestations.

  • Provide support to the Virtual IDT and serve as the manager on call during non-operational hours, handling follow-ups, documentation, and initiating grievances on behalf of participants or caregivers as needed.

  • Manage clinical delivery reports and represent Community Health Nurses (CHNs) during IDT meetings when necessary.

  • Allocate and balance staffing resources to ensure seamless, effective care delivery across the service area.

  • Maintain continuous communication with the IDT and other relevant parties to coordinate care aligned with participants’ health goals.

  • Collaborate with clinicians to review UAS assessments and ensure data accuracy within the UAS analyzer system.

  • Provide leadership and guidance to the CHN team, supporting clinical excellence and adherence to care standards.

Qualifications:

Education:

  • Degree from an NLN-accredited nursing school (RN required; BSN preferred, or a minimum of 5 years’ experience in a healthcare setting).

  • Completion of an accredited nursing program.

  • Completion of NYSED-approved infection control coursework (minimum of 8 contact hours, may include mandatory training).

  • Case Management Certification is recommended within 2 years of employment.

Experience:

  • Minimum of 2 years of administrative or management experience in CHHA, LHCSA, acute care, medical-surgical, critical care, or similar healthcare settings.

  • Prior supervisory experience required.

  • At least 1 year of experience working with the elderly population.

  • Experience in customer service and managed long-term care insurance is a plus.

  • Diagnostic and treatment clinic or nursing home experience preferred.

Licensure:

  • Current, active, and unrestricted Registered Nurse license in New York State is required.

Language Skills:

  • Bilingual candidates are preferred.
