Occupational Therapist (OT - Home Health)

Full-Time | Direct Hire | Brooklyn, NY (Williamsburg, Coney Island, Flushing)

Our healthcare client in Brooklyn, NY is seeking multiple Occupational Therapists to join their growing team. These are full-time positions offering a comprehensive benefits package. The roles are primarily on-site, with occasional fieldwork in participants' homes or care facilities.

The ideal candidate will have at least 1 year of experience as an Occupational Therapist and a strong background working with the elderly population. Please note: A personal vehicle is required for travel to participant locations.

Key Responsibilities:

  • Conduct thorough evaluations of participants referred by healthcare providers and develop individualized treatment plans in collaboration with the care team.

  • Perform assessments, progress evaluations, treatments, revisits, and discharge summaries, ensuring all documentation meets departmental standards and policies.

  • Provide therapy services both on-site and in the field, including participants' homes and nursing facilities.

  • Maintain a clean, safe environment and adhere to all safety protocols and procedures.

  • Participate actively in the development of participant care plans, collaborating with interdisciplinary team members and attending care plan meetings as required by state and federal regulations.

  • Accurately document participant evaluations, progress notes, Minimum Data Set (MDS), care plans, and statistical data according to established guidelines.

  • Consult with physiatrists and other healthcare professionals as appropriate to optimize patient care.

  • Utilize OT modalities and equipment effectively to enhance patient outcomes.

Qualifications:

  • Current New York State licensure as a Registered Occupational Therapist.

  • Minimum of 1 year of experience as an Occupational Therapist; experience with the elderly or frail population strongly preferred. (If no prior experience with elderly patients, training will be provided by the client.)

  • Strong ability to build rapport with patients, caregivers, and interdisciplinary staff.

  • Proficiency in using occupational therapy modalities and equipment.

  • Ability and willingness to travel to homes, nursing facilities, and clinical sites as needed (personal vehicle required).

  • Familiarity with EMR systems such as Care Compass is a plus.

  • Excellent documentation and organizational skills.
