Occupational Therapist (OT - Home Health)
Full-Time | Direct Hire | Brooklyn, NY (Williamsburg, Coney Island, Flushing)
Our healthcare client in Brooklyn, NY is seeking multiple Occupational Therapists to join their growing team. These are full-time positions offering a comprehensive benefits package. The roles are primarily on-site, with occasional fieldwork in participants' homes or care facilities.
The ideal candidate will have at least 1 year of experience as an Occupational Therapist and a strong background working with the elderly population. Please note: A personal vehicle is required for travel to participant locations.
Key Responsibilities:

Conduct thorough evaluations of participants referred by healthcare providers and develop individualized treatment plans in collaboration with the care team.

Perform assessments, progress evaluations, treatments, revisits, and discharge summaries, ensuring all documentation meets departmental standards and policies.

Provide therapy services both on-site and in the field, including participants' homes and nursing facilities.

Maintain a clean, safe environment and adhere to all safety protocols and procedures.

Participate actively in the development of participant care plans, collaborating with interdisciplinary team members and attending care plan meetings as required by state and federal regulations.

Accurately document participant evaluations, progress notes, Minimum Data Set (MDS), care plans, and statistical data according to established guidelines.

Consult with physiatrists and other healthcare professionals as appropriate to optimize patient care.

Utilize OT modalities and equipment effectively to enhance patient outcomes.
Qualifications:

Current New York State licensure as a Registered Occupational Therapist.

Minimum of 1 year of experience as an Occupational Therapist; experience with the elderly or frail population strongly preferred. (If no prior experience with elderly patients, training will be provided by the client.)

Strong ability to build rapport with patients, caregivers, and interdisciplinary staff.

Proficiency in using occupational therapy modalities and equipment.

Ability and willingness to travel to homes, nursing facilities, and clinical sites as needed (personal vehicle required).

Familiarity with EMR systems such as Care Compass is a plus.

Excellent documentation and organizational skills.
