The Rehabilitation Specialist is responsible for coordinating and delivering restorative and rehabilitative services to individuals at the Center and in their homes. Working alongside the Primary Care Physician, rehabilitation team, and other members of the interdisciplinary team (IDT), the Rehabilitation Specialist strives to enhance participant independence, safety, and functional abilities.

Key Responsibilities:

  • Perform assessments to determine the individualized needs, preferences, and goals for Occupational Therapy care, actively participating in IDT meetings to develop comprehensive care plans
  • Provide and document interventions in alignment with physician orders and agreed-upon care plans, utilizing techniques, devices, and activities to improve muscle function, cognitive abilities, balance, endurance, and independence in performing daily activities
  • Supervise and train Therapy Assistants, ensuring the implementation of Functional Maintenance Programs (FMP)
  • Deliver training to staff, participants, and caregivers/families on various Occupational Therapy areas relevant to their interactions with participants
  • Collaborate with IDT members to facilitate smooth care transitions between different settings, such as hospitals, skilled nursing facilities, and home environments. Also, participate in end-of-life care coordination and support


  • Bachelor’s Degree in Occupational Therapy from an accredited program following AOTA & ACOTE standards
  • Minimum of two (2) years of occupational therapy experience in a clinical setting, preferably working with a frail or elderly population
  • Two (2) years of supervisory experience, demonstrating the ability to mentor and develop team members, is preferred
  • Registration from the National Board for Certification in Occupational Therapy
  • Licensure or eligibility for licensure as required by the state of employment
