A regional medical group is hiring a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join an established primary care team in Apple Valley, California. Working under the direction of the Medical Director, this clinician delivers comprehensive primary care, manages a designated patient panel, and coordinates treatment plans to ensure strong continuity of care.
Schedule
Full-time, on-site, Monday through Friday, 8:00 AM to 5:00 PM.
Compensation
$155,000 to $170,000 per year.
What You'll Do
The Advanced Practice Provider performs comprehensive physical examinations, obtains detailed medical histories, establishes diagnoses, and manages evidence-based treatment plans for assigned primary care patients. This role orders diagnostic tests, interprets clinical results, and refers patients to specialized services as needed, collaborating closely with case managers to support continuity of care. The provider maintains high clinical practice standards, applies Hierarchical Condition Category (HCC) coding requirements, and participates in organizational quality and performance improvement initiatives. This role also leads sensitive end-of-life and hospice discussions with patients and family members when clinically appropriate, and builds positive working relationships across clinical departments through active participation in provider and staff meetings.
Required Qualifications
- Master of Science in Nursing (MSN), Doctorate of Nursing Practice (DNP), or completion of an ARC-PA accredited Physician Assistant program
- Active, unrestricted Nurse Practitioner or Physician Assistant license in the State of California
- Active, unrestricted DEA registration
- Board certification through AANP or ANCC (for NPs), or NCCPA (for PAs)
- Minimum of 2 years of experience as an Advanced Practice Provider in a Primary Care or Outpatient Family Medicine setting
- Strong knowledge of primary care, preventive health, and managed care concepts
Preferred Qualifications
- Experience with HCC coding and quality/performance improvement initiatives
- Experience leading end-of-life or hospice care discussions
- Experience working within a multidisciplinary care team
