HOSPICE REGISTERED NURSE CASE MANAGER
Dallas-Fort Worth, TX | $85,000 to $90,000 per year

A hospice admission is often the first point of contact where a patient and family begin to feel supported through a difficult transition. A well-established, locally operated North Texas hospice provider is seeking an experienced Hospice Admissions RN to serve as the initial clinical contact for prospective patients and families across the Dallas-Fort Worth market. This role reports to the Clinical Manager, with additional support from the Chief Operating Officer.

Schedule
This is a full-time position with schedule flexibility based on patient admission needs, limited on-call responsibilities, and regular local travel throughout Collin, Dallas, Denton, Tarrant, and Cooke counties.

Compensation
This role offers a base salary of $85,000 to $90,000 annually, based on experience and qualifications, plus mileage and cell phone reimbursement (details below).

What You'll Do
The Admissions RN evaluates prospective patients to determine clinical eligibility for hospice services, reviewing diagnoses, medical history, current symptoms, functional status, and supporting documentation. This includes explaining hospice philosophy, services, levels of care, and benefits clearly and compassionately to patients, families, and caregivers, and guiding them through emotionally difficult conversations with calm and respect.

The role completes all required admission assessments, consents, and documentation accurately and promptly, and initiates individualized plans of care based on each patient's clinical condition, symptoms, and immediate needs. The Admissions RN coordinates medications, equipment, and supplies needed for a safe transition into hospice care, and works closely with the Clinical Manager and interdisciplinary team to ensure a seamless onboarding process for every new patient.

The position also involves communicating significant clinical findings and condition changes promptly to the appropriate team members, maintaining documentation in accordance with company, Medicare, and regulatory requirements, and managing daily admission activity independently while staying in consistent contact with clinical leadership.

Required Qualifications

  • Current, unrestricted Texas Registered Nurse (RN) license
  • Minimum of one year of hospice nursing experience
  • Working knowledge of hospice eligibility requirements and the admission process
  • Current BLS/CPR certification
  • Valid driver's license, reliable personal transportation, and current automobile insurance
  • Ability to travel throughout the Collin, Dallas, Denton, Tarrant, and Cooke county service area
  • Ability to participate in limited on-call responsibilities

Preferred Qualifications

  • Experience with the Consolo electronic medical record system
  • Strong clinical assessment and critical thinking skills in emotionally sensitive settings
  • Working knowledge of Medicare hospice eligibility criteria and levels of care
  • Strong documentation, time management, and interdisciplinary collaboration skills

Compensation and Benefits

  • Base salary of $85,000 to $90,000 annually, based on experience and qualifications
  • Mileage reimbursement at $0.65 per mile
  • Cell phone reimbursement of $60 per month
