Facility Information: Nestled within the heart of Tupper Lake, our skilled nursing facility is devoted to delivering top-tier healthcare services and rehabilitation to residents. Responsibilities:

  • Administer physical therapy treatments in accordance with prescribed protocols.
  • Maintain accurate written assessments, progress notes, and discharge summaries for residents.
  • Collaborate with other healthcare professionals to optimize residents' rehabilitation or maintenance of function.
  • Participate in Resident Care Planning meetings and assume the Director of Physical Therapy role when necessary.
  • Maintain statistical records of residents' therapy treatments.
  • Assist with in-service training programs as needed.
  • Record and update residents' care plans in the Inter-disciplinary Kardex.
  • Submit supply requisitions as required.
  • Fulfill the duties of the Director of Physical Therapy in their absence, including supervision of Physical Therapy Aides and necessary documentation.

Qualifications:

  • Completion of an accredited Rehabilitation Specialist program.
  • Valid state license to practice as a Rehabilitation Specialist.
  • New graduates are encouraged to apply.
  • Preference for two years of experience in rehabilitation therapy.
  • Sincere interest in creating and participating in rehabilitation treatment programs for geriatric and subacute populations.
  • Demonstrated ability to lead and inspire others.
