$105-120K + Bonus + Paid Relocation + $20,000 Sign On Bonus

The Physical Therapist will be responsible for providing restorative and rehabilitative Physical Therapy services to participants at the center and in their homes. Collaborating with the Primary Care Physician, rehabilitation team, and other members of the Interdisciplinary Team (IDT), the Physical Therapist will assist in enhancing participant independence, safety, and functionality.

Key Responsibilities:

  • Perform Physical Therapy assessments at the facility and in participants' homes to determine their care needs, preferences, and goals
  • Engage actively in IDT meetings to develop individualized care plans for participants
  • Implement and document Physical Therapy interventions according to physician orders and/or participants' care plans
  • Administer active and passive exercises, muscle re-education, gait training, and functional training for activities of daily living; provide treatments such as massage, heat, sound, water, light, or electricity
  • Supervise Therapy Assistants, providing necessary training and guidance for the implementation of Functional Maintenance Programs (FMP)
  • Train staff, participants, and caregivers/families on the use and maintenance of therapeutic equipment

Minimum Qualifications:

  • Bachelor's Degree in Physical Therapy with at least five (5) years of experience in the field
  • Master's Degree or Doctorate in Physical Therapy preferred
  • Valid California Physical Therapy License or eligibility for licensure
  • Two (2) years of experience in a clinical setting with frail or elderly populations preferred
  • Evaluation of non-US physical therapy degrees for equivalence to US degrees
  • CPR certification preferred
