PHYSICAL THERAPIST

We are seeking a dedicated and experienced Physical Therapist who will play a crucial role in providing restorative and rehabilitative Physical Therapy services to participants both at our center and in their homes. Working closely with the Primary Care Physician and the rehabilitation team, the Physical Therapist will strive to maximize participant independence, safety, and functional abilities.

Key Responsibilities:

• Perform thorough Physical Therapy assessments at our facility and in participants' homes to identify their care needs, preferences, and goals
• Collaborate actively in interdisciplinary team (IDT) meetings to develop comprehensive care plans for participants
• Deliver and document evidence-based Physical Therapy interventions according to physician orders and/or agreed-upon care plans
• Provide a range of interventions, including active and passive exercise, muscle re-education, gait training, and functional training for activities of daily living (ADLs)
• Supervise and provide guidance to Therapy Assistants, ensuring the effective implementation of Functional Maintenance Programs (FMP)
• Offer training and instructions on the proper use and maintenance of therapeutic equipment to staff, participants, and caregivers/families

Requirements:

• Bachelor's Degree in Physical Therapy with a minimum of five (5) years of experience
• Master's Degree or Doctorate in Physical Therapy preferred
• Valid California Physical Therapy License or eligibility for licensure
• At least two (2) years of experience in a clinical setting, preferably with a frail or elderly population
• If physical therapy degree obtained outside of the US, the degree must be equivalent to a US degree in physical therapy
• Current CPR certification is preferred
