Hospice RN Case Manager

A hospice care provider serving the Kansas City metro area is hiring a full-time RN Case Manager. This role is responsible for coordinating and overseeing an assigned caseload of hospice patients, developing individualized plans of care, and providing ongoing clinical assessment and symptom management. The case manager works closely with the interdisciplinary care team to support patients and families throughout the course of care.

Schedule
Full-time. Field-based role covering the Kansas City metro area (east side). 

Compensation
$85,000 to $95,000 per year.

What You'll Do
This nurse coordinates and oversees all aspects of care for an assigned caseload of hospice patients, developing and implementing individualized plans of care in collaboration with the interdisciplinary team. The role includes performing head-to-toe physical assessments, monitoring symptoms, and managing medications to keep patients comfortable throughout their course of care. The case manager educates patients and families regarding their condition, needs, and care options, communicates patient progress to clinical leadership, physicians, and the broader care team, and completes all required documentation accurately and within required timeframes.

Required Qualifications

• Current Registered Nurse (RN) license valid for practice in Missouri
• Two years of RN experience in a clinical care setting
• Current CPR certification
• Valid driver's license, reliable transportation, and valid automobile liability insurance
• Ability to work independently and exercise sound clinical judgment
• Strong written, verbal, organizational, and interpersonal communication skills

Preferred Qualifications

• One year of full-time hospice experience
• Certified Hospice and Palliative Nurse (CHPN) certification
• Familiarity with hospice documentation and electronic medical record systems
