Hospice RN Case Manager, South Dallas Territory

Location: Dallas, TX (Field-based, South Dallas territory)
Job Type: Full-time
Pay: $85,000 to $95,000 per year

Guide patients and families through end-of-life care with compassion, clinical skill, and the support of a strong interdisciplinary team.

Must-Haves

• Active RN license valid for practice in Texas
• 2+ years of RN experience in a clinical care setting
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

What You’ll Do

• Coordinate and oversee all aspects of assigned patients’ hospice care
• Develop and implement individualized plans of care with the interdisciplinary team
• Perform head-to-toe assessments, monitor symptoms, and manage medications
• Provide symptom management and comfort-focused care
• Educate patients and families on condition, needs, and care
• Communicate progress with physicians and clinical leadership
• Complete accurate, timely documentation

Nice-to-Haves

• 1+ year of full-time hospice experience
• Home health or palliative care experience
• CHPN certification

Compensation and Benefits

• $85,000 to $95,000 per year
