Hospice Admissions RN

A hospice care provider serving the Kansas City metro area is hiring a full-time Admissions RN. This role is responsible for coordinating new patient admissions, completing comprehensive clinical assessments, and educating patients and families as they transition into hospice care. The nurse will work closely with an interdisciplinary care team to support continuity of care from the point of referral forward.

Schedule
Full-time. Field-based role covering the Kansas City metro area (east side). Specific shift and on-call rotation details should be confirmed with the hiring team.

Compensation
$85,000 to $95,000 per year.

What You'll Do
This nurse coordinates and oversees all aspects of new patient admissions to hospice services, working directly with patients and families in their homes or care settings to explain the hospice process and what to expect. The role includes performing head-to-toe physical assessments and comprehensive intake evaluations, then documenting findings accurately and within required timeframes. The Admissions RN collaborates closely with the interdisciplinary care team, physicians, and other hospice staff to ensure smooth handoff and continuity of care after admission, and communicates clearly with patients and families about their loved one's condition, needs, and available services throughout the admissions process.

Required Qualifications

• Current Registered Nurse (RN) license valid for practice in Missouri
• Two years of RN experience in a clinical care setting
• One year of full-time hospice experience
• Current CPR certification
• Valid driver's license, reliable transportation, and valid automobile liability insurance
• Ability to work independently and exercise sound clinical judgment
• Strong written, verbal, organizational, and interpersonal communication skills

Preferred Qualifications

• Certified Hospice and Palliative Nurse (CHPN) certification
• Prior experience specifically in hospice admissions or intake
• Familiarity with hospice documentation and electronic medical record systems

Compensation and Benefits
