Home Health Registered Nurse, OASIS Case Manager

Location: Fort Worth, TX (Field-based, Tarrant County area)
Job Type: PRN
Pay: $85,000 to $92,000 per year

Manage your own home health caseload in the field, delivering skilled nursing care where patients need it most.

Must-Haves

• Active Texas RN license in good standing
• Recent home health RN case management experience
• Strong knowledge of Medicare home health, OASIS, and plan-of-care management
• Strong clinical judgment, documentation, and time-management skills
• Ability to work independently while communicating with the care team
• Valid driver’s license and reliable transportation

What You’ll Do

• Manage an assigned home health caseload in the field
• Perform skilled nursing visits, assessments, recertifications, resumptions of care, and discharges
• Develop and coordinate individualized plans of care with physicians, patients, and families
• Complete accurate, timely OASIS and visit documentation
• Monitor patient progress and communicate changes in condition
• Educate patients and caregivers on disease management, medications, and safety
• Coordinate care with therapy, social work, aides, and physicians
• Maintain compliance with home health regulatory and quality standards

Nice-to-Haves

• EMR experience
• Experience in Medicare-certified home health agencies

Compensation and Benefits

• $85,000 to $92,000 per year
