Territory Clinical Manager, Home Health RN

Location: San Diego, CA (Travel to all territory locations)
Job Type: Full-time
Pay: $110,000 to $135,000 per year
Relocation: Assistance available

Lead patient care services and clinical staff for a home health territory, driving quality, compliance, and great patient outcomes.

Must-Haves

• Graduate of an approved school of professional nursing
• Active California RN license
• 3 to 5 years of RN experience
• Current CPR certification
• Valid driver’s license and insured, reliable vehicle

What You’ll Do

• Oversee all patient care services and clinical personnel
• Manage day-to-day clinical and administrative office operations
• Supervise and coach clinical staff
• Coordinate intake, referrals, and individualized care plans with the interdisciplinary team
• Review discharge assessments and edit and lock Discharge OASIS forms
• Lead QAPI, track incidents, analyze trends, and implement corrective actions
• Maintain CHAP accreditation and regulatory survey readiness
• Manage HHCAHPS survey distribution and patient satisfaction
• Lead emergency preparedness planning
• Cover for the Territory Administrator and other clinical leaders as needed

Nice-to-Haves

• Experience managing RN services in home health, community, or clinic settings
• BSN
• SHP analytics experience

Compensation and Benefits

• $110,000 to $135,000 per year
